Academic Assessment Reports & Forms

Assessment plans are expected to vary among departments in both content and length. The purpose of this online form is to ensure that the material on file centrally is submitted in a concise, consistent format. The form will produce a web document that will serve as the published version of each departmental plan. Departments are expected to have more detailed information on file. Program faculty review the Assessment Plan annually and modifications are made as needed based on information in the Annual Program Report. Click on  the Upload Program Assessment and Annual Report link (bottom of page). Next, enter your Username and Password (contact Sally Munday @ 683-7121 or email s_munday@umwestern.edu for this information).

Complete the Program Assessment Report and Annual Report forms in one of two ways:   

• Fill in the boxes interactively. As more information is entered, the lines will scroll up; use the scroll bar on the right side of the box to read text that has scrolled out of sight. Use the [Save] button at the bottom of the form to save work and return to it at a later time.

   

• Type information using a word processing program. Then copy and paste the paragraphs into the proper boxes by using the Edit menu.

In both cases, provide information in each of the boxes.

Use the [SAVE] and then [WEB SITE] buttons to see what the finished document will look like. Print the page to make a printed copy of your document.

When the [SAVE] button at the end is clicked, both the form contents and the finished web document will be saved. Make corrections by using the [CONTINUE EDITING] button to recall the form and then clicking the [SAVE] button again. Questions can be directed to Karl Ulrich at 683-7151 or k_ulrich@umwestern.edu.

The Accreditation Coordinator will upload Self-Study Narratives and reports provided by external reviewers to this site as PDF (Adobe) documents. To access posted reports, click on the link associated with the program located under either the Self-Study Narrative column or External Reviewer’s Report column.
